How to Use This Diagnostic
This diagnostic is divided into four scored sections. For each of the 19 questions, rate yourself honestly using the scale below. Then use the reflection space to write a brief note, prayer, or next step. Your scores and reflections will be shared with Pastor as the foundation for your one-on-one conversation — answer as honestly as you would if Pastor were sitting across from you, because that conversation is coming. There are no right or wrong answers; the goal is honest self-examination before God and your leadership.
Use this as your guide as you rate each question. Your scores will be seen by Pastor — let that sharpen your honesty, not soften it.
